I/We the parent(s) or guardian(s) of the above named registrant hereby give my/our approval to her/his participation in any type of softball activities during the current season and agree to abide by the rules set forth by South Bow River Softball. I/We do further hereby waive, indemnify, release, absolve and agree to hold blameless South Bow River Softball, it’s employees and program sponsor, the officers of South Bow River Softball, the organizers, coaches, managers and persons transporting my child to or from activities for any claim arising out of an injury to my child, whether the result of negligence or for any other cause, except to the extent covered by Softball Alberta Insurance. Activities for example include softball evaluations, games, exhibitions, tournaments, practices and/or clinics whether adult attended or not. Please note there is an administration fee of $25 for any cancelled registration.
